Income Protection for Mental Health Nurses

A general ward nurse's main physical risk is lifting. A mental health nurse's is a person.

Restraint and physical intervention are part of the role, carried out at speed in corridors and day rooms against resistance, with no choice about position. Assault on staff is a recognised risk in this field. And unlike a police officer or a prison officer, the nurse has to rebuild a therapeutic relationship with that patient the next day, because care rather than custody is the point.

Income protection pays a monthly amount while you cannot work. For this speciality the cover has to respond properly to psychological causes, because that is what the absence data reflects.

    The short answer

Mental health nursing carries an assault and restraint risk alongside a sustained emotional load, and the two interact in a way that is specific to this field: you have to maintain a therapeutic relationship with the same patient afterwards. Long-term absence in this speciality is dominated by mental health conditions rather than by physical injury.

    What actually stops you working

    • Mental health conditions including post-traumatic stress, which account for the largest share of long-term absence in this speciality
    • Assault by patients, which is a recognised occupational risk in mental health settings
    • Musculoskeletal injuries from restraint and physical intervention, carried out at speed without choosing your position
    • Burnout from sustained emotional load, caseload pressure and staffing shortfalls
    • Exposure to self-harm and to patient deaths, with a cumulative rather than a single effect
    • Sleep disorders and fatigue from long shifts and night working

Sick pay, if you get any

Mental health nurses in the NHS have occupational sick pay escalating with length of service: a period at full pay, then a period at half pay, then nothing beyond statutory sick pay. It is a good scheme by the standards of this site and is normally counted across a rolling period, which matters because absence in this speciality is often repeated rather than single. Nurses in independent sector hospitals have whatever their contract provides, which varies.

Why the definition of incapacity decides it

The clause matters because of how absence here presents. A nurse signed off after an assault, or with burnout, is frequently capable of other work including other nursing, and an any occupation definition will find them fit. An own occupation definition asks whether you can do mental health nursing, which means returning to the same setting and the same therapeutic demands.

You Have To Keep The Relationship Afterwards

This is the thing that separates mental health nursing from every other job on this site that involves confrontation, and it is worth stating directly.

A police officer restrains somebody and the relationship ends there. A prison officer manages the same population but within an explicitly custodial frame. A mental health nurse may be involved in restraining a patient and then has to provide their care, build trust and work therapeutically with them the following day, because that is what the job is for. Holding both of those at once is a genuine psychological load and it is the part that accumulates.

Income protection responds to being unable to work whatever the cause, so mental health conditions are assessed on the same basis as a physical injury. The practical consequence is the usual one with more weight than usual: anything already recorded in your notes when you apply will be asked about, which makes arranging cover during a settled period materially better than during a difficult one.

Restraint Injuries Are Manual Handling Without The Handling Rules

The physical side of this role is real and does not look like a manual job, which is why it is underestimated.

Physical intervention happens quickly, in whatever space is available, against a person who is resisting, with no opportunity to adopt a safe position or protect your back. The shoulder, knee and back injuries that follow are the same injuries a manual trade produces, reached by a completely different route and with none of the controls a lifting task would have.

They also recur, because returning to the role means being available for the same situations. Combined with a rolling sick pay entitlement, that is a strong argument for arranging cover early in a career rather than after the first incident.

    If you stop

The shift is covered, often by agency staff, and the post is generally held, so this is financial rather than an employment risk. What is specific is the return: going back may mean the same ward and sometimes the same patients, which is why absences in this field are more likely to repeat or to end in a move to a different setting than in a clean return to the same job.

NHS Death In Service, And The Usual Gap

Nurses in the NHS pension scheme generally have death in service, which is genuine cover and a proper starting point. The figure is on your own benefit statement.

Two checks apply. It is tied to scheme membership, so leaving or opting out changes it, and it is calculated on pensionable pay, which may not include all the unsocial hours and enhancements that make up what the household actually lives on.

Compare the figure with the mortgage and the years dependants would need supporting, and arrange the difference. For most households that gap exists even with a good scheme behind them.

A Registration That Opens Other Doors

The common ending in this speciality is not disability. It is deciding, or being advised, that you cannot continue in this setting.

That person holds a valuable registration and has real options: community rather than inpatient work, education, liaison roles, or a different speciality. Several involve a change in income, at least initially.

A lump sum on diagnosis funds that move, and it pays regardless of whether you are judged capable of other work, which is precisely the position somebody leaving this speciality is in.

    Common questions

How is this different from general nursing cover?+

The product is the same and the risks diverge sharply. General ward nursing is dominated by lifting and musculoskeletal injury. Mental health nursing is dominated by assault, restraint and the psychological load of maintaining a therapeutic relationship with somebody you may have had to restrain.

Would an absence after an assault be covered?+

Yes, income protection responds to being unable to work through illness or injury, and that includes the psychological aftermath as well as any physical injury. Mental health conditions are the largest cause of long absence in this speciality and are assessed on the same basis as anything else.

I have NHS sick pay. Why would I need this?+

Because it steps down to half pay and then stops, and because it is counted across a rolling period. In a speciality where absences often repeat, a lot of the entitlement can already be used before the long absence arrives, so the figure on paper is frequently not the figure available.

Would a restraint injury be covered?+

Being unable to work through injury is the test, however it happened. Restraint injuries are the same shoulder, knee and back problems a manual trade produces, and they tend to recur because returning to the role means being available for the same situations.

What if I decide I cannot go back to this setting?+

That is a common ending and it is where income protection helps least, because it pays while you cannot work rather than funding a change of direction. Critical illness cover provides a lump sum on diagnosis, which makes a move into community work, education or another speciality affordable.
