Income Protection for Midwives
The job is often described as rewarding, which is true and tends to obscure what it does to a body and a sleep cycle.
Births happen when they happen. Continuity models and on-call rotas mean being called out at any hour and then sometimes working through, and a long labour is many hours of sustained physical support and concentration with no natural end point. Community work adds the domestic environment, where the position you work in is whatever the room allows.
Income protection pays a monthly amount while you cannot work. NHS sick pay covers the first stretch, so the exposure is what comes after it.
The short answer
Midwifery differs from ward nursing in two ways that matter. Labour does not schedule itself, so continuity and on-call working produce sustained sleep disruption rather than a predictable shift pattern. And community and home birth work means supporting women on domestic beds, bathroom floors and birth pools, where none of the hoists and adjustable beds a ward has are available.
What actually stops you working
- Lower back and shoulder injuries from supporting women in domestic settings, on low beds, bathroom floors and around birth pools
- Knee and hip problems from kneeling and crouching at floor level for long periods during labour
- Chronic fatigue and sleep disorders from on-call working and being called out at any hour
- Mental health conditions, where the emotional weight of poor outcomes and the litigation climate in maternity both contribute
- Post-traumatic stress following obstetric emergencies, which happen suddenly and with very high stakes
- Infections and exposure risks inherent in clinical and intrapartum care
Sick pay, if you get any
Midwives in the NHS have occupational sick pay escalating with service: full pay for a period, then half pay, then nothing beyond statutory sick pay. It is a good scheme by the standards of this site. The practical point is that it is usually calculated on pensionable pay, while on-call payments and unsocial hours enhancements can be a meaningful part of what a midwife actually earns, which is worth checking against your own payslips.
Why the definition of incapacity decides it
The clause matters because the role requires more than general fitness: supporting a labouring woman in an uncontrolled environment, kneeling for long periods, and being available on call. A midwife with a back injury can do a great deal of other clinical work, which an any occupation definition will treat as fitness. An own occupation definition asks whether you can work as a midwife.
A Home Birth Has None Of A Ward's Equipment
This is the physical mechanism that separates community midwifery from ward nursing, and it is the same problem a paramedic has by a different route.
A labour ward has beds that raise and lower, hoists where needed, and colleagues within reach. A bedroom has a divan at the wrong height, a bathroom has a floor and a toilet in the way, and a birth pool has to be worked around at a crouch. The support still has to be given, for hours, in whatever position the room permits.
That produces back, shoulder, knee and hip problems at a rate the job title does not suggest. Because they build gradually they are often documented before a midwife looks at insurance, and anything already recorded when you apply will be asked about. Arranging cover early in a career is worth more than arranging it at forty-five.
On Call Is The Part That Accumulates
Shift work is one thing. Being on call is another, because you cannot plan rest around a phone that may not ring.
Continuity models and on-call rotas mean broken sleep whether or not you are called, then potentially a long labour and sometimes a working day afterwards. Sustained over years that is associated with chronic fatigue and sleep disorders, and fatigue contributes to the musculoskeletal injuries above as well as being a problem in itself.
There is a financial dimension too. On-call payments and enhancements can form a real part of a midwife's earnings while sick pay and death in service are often calculated on pensionable pay. Somebody insuring their basic has arranged cover against a figure their household does not live on, which is worth settling before the policy rather than during a claim.
If you stop
The caseload is redistributed and the post is generally held, so this is a financial rather than an employment problem. What is specific to continuity models is that women booked with you have a relationship with you, and redistributing a caseload mid-pregnancy is harder on everybody, which is one reason midwives return from absence sooner than they should.
A Good Scheme, Calculated On Less Than You Earn
Midwives in the NHS pension scheme generally have death in service, which is real cover and worth getting the exact figure for from your benefit statement.
The detail that matters is the basis. A multiple of pensionable pay may exclude on-call payments and unsocial hours enhancements, so the cover can be smaller relative to the household's actual spending than the headline multiple suggests.
Compare it with the mortgage and the years your family would need supporting. Where there is a gap, arranging the difference is usually inexpensive for somebody in good health.
Capital For Midwifery Without The On Call
The characteristic ending is a back that cannot support labours in domestic settings any more, or a body that cannot sustain on-call working, in somebody with a valuable registration.
There are destinations that keep the qualification in use: antenatal and postnatal clinic work, education, specialist roles, screening and public health midwifery. Several are less well paid, particularly without the on-call enhancements.
A lump sum on diagnosis makes that move affordable. Income protection pays while you cannot work and is not designed for the case where you can work, but not in this pattern.
Common questions
How is this different from cover for a nurse?+
Two things. Labour does not schedule itself, so on-call working and broken sleep are structural rather than a shift pattern. And community and home birth work means supporting women on domestic beds and floors, with none of the adjustable beds or hoists a ward provides.
I have NHS sick pay. Do I need this?+
It is a good scheme and it steps down to half pay and then stops. Cover is usually set to start around the point your pay reduces, which keeps the premium lower. The thing to check is what the scheme is calculated on, because on-call and enhancements may not be included.
Should I insure my on-call payments?+
Worth discussing, because for many midwives they are a real part of what the household lives on while employer arrangements are based on pensionable pay. How variable income is treated differs between insurers, so we would look at your actual payslips rather than give you a rule.
Would post-traumatic stress after an emergency be covered?+
Yes, income protection responds to being unable to work through illness including mental health conditions. Obstetric emergencies happen suddenly and with very high stakes, and the aftermath is a recognised cause of absence. Anything already in your records when you apply will be asked about.
Would a back injury from a home birth be covered?+
Being unable to work through injury is the test, however it happened. The definition matters: own occupation asks whether you can work as a midwife, which involves supporting labours in uncontrolled environments, while an any occupation policy may find you fit for other clinical work.
